People experiencing homelessness and structural vulnerability face a higher risk of illness and many gaps and barriers in accessing mainstream health care services. Currently, about 50% of Canadians are living paycheque to paycheque, demonstrating the increasing need for programs and services to help address structural vulnerability in our society.
At Journey Home Hospice, our goal is to provide high-quality hospice palliative care that is trauma-informed, culturally safe, and harm-reducing, ensuring that each patient’s individual needs and life story are honoured.
Our impacts are measured and evaluated in a number of key areas including quality of care, end-of-life experience, safety, and patient and staff satisfaction. We are also interested in tracking the health system and community impacts of Journey Home Hospice over time.
